Direct Primary Care is direct access to your doctor. No phone trees to get through or appointments to wait for; you contact and interact with your doctor when and how you need.
Patients pay a monthly or annual fee which will cover all the services provided in the office. Care includes physicals and health maintenance as well as acute care for illness and injury.

You’ll be contacting the doctor directly by email, text or phone to answer questions, coordinate care or arrange appointments. For in-person appointments, in many cases the doctor can see you that day or the next.
It takes time to listen and listening is key to diagnosis as well as making sure we are working toward YOUR healthcare goals. Appointments can be up to 1 hour long and there is no limit to how often you can be seen.
There are no co-pays or hidden costs. Wholesale pricing through local lab and imaging centers allows those without insurance the best deals.
Shady Grove Direct Care is a primary care practice providing full-service office-based Family Practice care for patients aged 5-100+.
Membership includes access to Dr. Cronin by email, text, or phone 24/7. She will do her best to respond that day or the next with a response or appointment.
Office hours are Monday, Tuesday and Thursday, 9:00-3:00. The office is located in Downtown Eugene in the historic Smeede Hotel Building, 767 Willamette, Suite 303.
When appropriate, care can be arranged by tele-visit. Home visits and office visits during non-regular office hours, holidays or weekends are possible by special appointment.
The monthly or annual fee includes unlimited office visits per year
Most procedures that are done in the office are covered; please see services section for a detailed list of services. There are no co-pays for office visits.
Fees do not cover costs of lab tests, imaging studies, specialist or hospital fees including emergency or urgent care.
-Fees may be paid monthly or annually* and are based on age.
-Fees will be transmitted on-line and no cash will be handled.
-There is a one-time enrollment fee of $100 per patient that covers collection and review of records.
-If you cancel and wish to re-enroll, you will be charged a re-enrollment fee of 2 times your monthly rate.
*discounts are available for pre-payment of year’s fees. Discounts also available for members of the same immediate family
You can cancel your membership at any time, which will be terminated at the end of the last calendar month paid. If you have paid for the whole year and decide to cancel, you are eligible for a refund for the remainder of your term. There are no charges for cancellation of membership.
