What is Direct Primary Care?

Direct Primary Care is direct access to your doctor. No phone trees to get through or appointments to wait for; you contact and interact with your doctor when and how you need.
Patients pay a monthly or annual fee which will cover all the services provided in the office. Care includes physicals and health maintenance as well as acute care for illness and injury.

You’ll be contacting the doctor directly by email, text or phone to answer questions, coordinate care or arrange appointments. For in-person appointments, in many cases the doctor can see you that day or the next.


It takes time to listen and listening is key to diagnosis as well as making sure we are working toward YOUR healthcare goals. Appointments can be up to 1 hour long and there is no limit to how often you can be seen.


There are no co-pays or hidden costs. Wholesale pricing through local lab and imaging centers allows those without insurance the best deals.


Shady Grove Direct Care is a primary care practice providing full-service office-based Family Practice care for patients aged 5-100+.

Membership includes access to Dr. Cronin by email, text, or phone 24/7. She will do her best to respond that day or the next with a response or appointment.

Office hours are Monday, Tuesday and Thursday, 9:00-3:00. The office is located in Downtown Eugene in the historic Smeede Hotel Building, 767 Willamette, Suite 303.

When appropriate, care can be arranged by tele-visit. Home visits and office visits during non-regular office hours, holidays or weekends are possible by special appointment.

The monthly or annual fee includes unlimited office visits per year

Most procedures that are done in the office are covered; please see services section for a detailed list of services. There are no co-pays for office visits.

Fees do not cover costs of lab tests, imaging studies, specialist or hospital fees including emergency or urgent care.

-Fees may be paid monthly or annually* and are based on age.

-Fees will be transmitted on-line and no cash will be handled.

-There is a one-time enrollment fee of $100 per patient that covers collection and review of records.

-If you cancel and wish to re-enroll, you will be charged a re-enrollment fee of 2 times your monthly rate.

Meet & Greet

We will schedule a brief "meet & greet" visit; this will allow you to interview Dr. Cronin in-person and help you determine if this membership practice is for you.

About Dr.Cronin

Dr. Kimberly Cronin has been practicing medicine for 30 years and has been here in Lane County for nearly 25 of those years. She most recently worked for Peacehealth from 2000-2021; through 2017, she worked as an Emergency Physician at Cottage Grove Medical Center where she was also Medical Co-Director. Additionally, she served as Quality Director for the Hospital and Emergency Department as well as serving as the founding Chair for the Patient and Family Advisory Council. In the last 4 years, she has worked in the Urgent Care offices of Peacehealth Medical Group.

Dr. Cronin's residency training was in Family Medicine, and she remains Board Certified through the ABFM. She has maintained this certification with its continuing education requirements and testing while working in the more emergent/urgent settings.

Dr Cronin has always had special interest in whole person, wellness care; in her first year of medical school, she was a founding member of the American Holistic Medical Association, Student Chapter. She believes that true Family Practice is indeed a path to Holistic Care. The common-sense tenets of Lifestyle Medicine and Blue Zone Living are in alignment with her personal views on health and wellness. She is open to (safe) Alternative, Integrative and Functional concepts while maintaining a "First Do No Harm", evidence-based approach.

No, we do not bill any insurance and you are not able to charge your insurance for anything done in the office either. We do, however, recommend that you carry insurance to use for labs, imaging, specialist/ER/hospital visits, etc. Even if Dr. Cronin is not in your insurance network, unless you have an HMO plan or Providence insurance, she can also still order labs, Xrays and refer you to specialists as long as they remain in network. If you do not have insurance, you can still join for a monthly fee. That being the case, we can offer you low-cost labs and imaging if you do not have insurance or have insurance with a high deductible.
Yes, however we will not bill Medicare for Dr. Cronin’s services, and you cannot bill them for Dr. Cronin’s services either. You will also be required to sign a waiver stating that you agree to not bill Medicare for services provided in the office.

You can cancel your membership at any time, which will be terminated at the end of the last calendar month paid. If you have paid for the whole year and decide to cancel, you are eligible for a refund for the remainder of your term. There are no charges for cancellation of membership.

Yes, I am accepting new patients ages 5 and up. Please click on the “Send Us a Message” tab here to send us an email that you are interested in joining. We would then schedule a no-charge “meet and greet” to ensure that this practice can meet your expectations.
Once you’ve been accepted as a patient, Dr. Cronin will review for with you the different ways to make an appointment. The majority of appointments will likely be made through email in the patient portal but more urgent issues may require more prompt scheduling by phone.
If you are having a medical emergency, call 911. For non-emergency questions or concerns you email, call or text. Dr. Cronin will try and respond the same or the very next day.
For non-emergency needs I am available at a reasonable hour by phone or email. I can even prescribe medications if needed, which you can pick up at a pharmacy near you, wherever that may be (within the United States).
No, you are not able to pay your membership dues with the above-mentioned plans. Please be advised to discuss this with your insurance carrier or your employer. Having said that you can use these plans to pay for your labs, imaging, and medications. Please click here for more information.

If you do not “believe” in the existence of Covid-19 and/or the use of vaccines to prevent serious illness and death, then it is unlikely that Shady Grove Direct Care would be able to meet your healthcare needs.
